COVID-19 â€“ Safety Statement

The safety and comfort of our clients and staff is our highest priority.

To ensure the safest possible environment, clients are being seen one at a time during this period, and physiotherapists will check temperatures before clinic entry is permitted.

Clients will also be asked to sanitise hands, remove shoes and wear a mask at all times.

We are also asking all clients to sign the below declaration that they are free from the virus.

Classes

We are currently restricting all classes to a maximum of 4 people, and holding them in one place - the clinic in Tennant Hall, Christchurch, Rathgar Road.

Clients enter one by one, masks will be worn and there will be screens separating clients.

The room will be cleaned after all classes, with 15 minutes allowed between classes for sanitisation.

DECLARATION

I, the undersigned, declare that Iâ€¦

Have NOT been diagnosed with confirmed or suspected COVID-19 infection in the last 14 days
Do NOT have a fever with a temperature above 37.5degrees Celsius
Do NOT have symptoms of cough, fever, high temperature, sore throat, runny nose, breathlessness, headache, flu-like symptoms or loss of sense of smell or taste now or in the past 14 days
Have NOT travelled internationally within the last 14 days
Am NOT a close contact of a person who is a confirmed or suspected case of COVID-19 in the past 14 days
Have NOT been advised by a doctor to self-isolate at this time
Have NOT been specifically advised by a doctor to cocoon at this time

I further declare that, if my answers change, or I start to suffer any of these symptoms, I will not come to class.

I acknowledge the contagious nature of the Coronavirus/COVID-19, the need to practice social distancing and the risks inherent in any gatherings at this time. I agree that PhysioNow has put in place preventative measures to reduce the spread of the Coronavirus/Covid-19, but I understand that PhysioNow cannot guarantee that I will not become infected with the virus.
